Health Special Risk, Inc. (HSR) continues to administer The student insurance program. HSR will continue to process and pay all claims from the new Plano, Texas office.
The District Master Policy is the official document providing parents with specific policy provisions, limitations, and exclusions pertaining to the program. Please review the policy thoroughly as this document will govern all aspects of the program, including claims adjudication.
The Claim Form has been updated. The P.O. Box has also changed this year so to ensure the fastest handling of all claims, please forward to the new address noted on this year's claim form:

School officials, trainers and coaches can continue to look up student enrollments as usual via the HSR FAST TRACK web site at https://www.hsri.com/K12FT. Cassandra Talton, 866-345-2680 ext. 5660 at HSR is your Fast Track point of contact, if you need assistance. Fast Track Users can submit claim forms electronically on behalf of your student athletes directly into HSR's adjudication systems to expedite these accidents when they occur.
For enrollment issues or claims inquires, the HSR Customer Service Department is available toll-free daily from 8:00 a.m. to 5:00 p.m. Central Time at 866-409-5733. Specialized enrollment personnel are standing by to assist parents and students with their enrollment needs or to answer any claims-related questions.
