COMPLETE THE ENCLOSED INFORMATION CARDS AND FORMS THAT YOU RECEIVED IN THE MAIL AND BRING THEM WITH YOU! (You will receive this packet in the mail this summer. If you do not receive one, please contact the Jr. High at 951.8975).

AGR FORM FILLED OUT AND SIGNED BY PARENT/GUARDIAN (RESPONSIBLE PERSON)

1. Sign Acceptable Use Agreement Cards (for computer and internet use)
2. Visit the nurse about necessary shot records/fill out emergency cards (An immunization clinic will be held on August 5th during JumpStart for those who need shots.)
3. Pay for overdue fines if necessary
4. Pick-up physical papers for athletics
5. School IDs will be made (dress code
6. Review Attendance  Policy
7. Receive schedule of classes
8. Get locker assignment and lock
9. Receive books
10. Complete lunch forms and have picture taken
11. Enrollment for ACE program available
12. Join Junior Hgh PTO (Membership $1.00)
13. Junior High T-Shirts for sale

Students will not be allowed to start the process until their scheduled time.  If you cannot come during your scheduled time, you may come at a time later in the day.
